Before filling out any paperwork, please contact Dr. Tang's office to verify travel range and also to verify insurance.

The New Patient Packet & the New Patient Questionnaire must be filled out entirely before the first appointment is scheduled. The Concierge membership is optional.

  Tell us about you! This Patient Questionnaire is a thorough overview of medical and social background. This gives Dr. Tang a better look at how to treat you best.
• Concierge Membership Agreement (Optional)
  If the Concierge Membership fits your needs best please include this Concierge Membership Agreement with your New Patient Packet.
